What services do you offer at your clinic?

We specialize in Medical Marijuana Certifications, Weight control, hormone therapy, treatments for anxiety, insomnia, and concentration medications for conditions such as ADHD.

Do you take insurance?​​​
We currently do not accept any insurance. It is a selfpay clinic.
Our office is a cashless office. Our office accepts Card Payments Only – No Cash or Checks Accepted

Appointments

​​Do I need an appointment, or do you accept walk-ins?
Appointments are required to ensure timely care, we do not accept walk-ins. You can book by calling us directly.

What should I bring to my first appointment?
​
Please bring a valid government-issued ID.

Are you accepting new patients?
​
Yes, Dr. deFreese is accepting new patients. He currently is only accepting both male & female patients for Medical Marijuana Certification. For all other services are available to women only.

Will I get a reminder about my appointment?
​
Everyone will receive an appointment reminder 1-2 business days before the appointment by our automated system email, text message and/or phone call.

🌿 Medical Marijuana Certifications
​
Who qualifies for a medical marijuana card?
Eligibility varies by state, but common qualifying conditions include chronic pain, anxiety, PTSD, insomnia, and more. We'll assess your condition and determine your eligibility during your visit.
​How long does the certification process take?
Most patients can complete their evaluation and submit their application in one visit. State processing times may vary.
Can I renew my medical marijuana card through your clinic?
Yes, we offer renewal evaluations for existing medical marijuana patients.
Will my insurance cover the certification?
Medical marijuana evaluations are not typically covered by insurance and are considered out-of-pocket.

​What if I need a refill on my prescription(s)?

If you are up to date with your appointments, ask the pharmacy to send a refill request to the office.
​
What if I need a refill of a controlled prescription?
​
To comply with medical guidelines and state regulations, patients receiving controlled substances will need to be seen more frequently than once a year.
Non-controlled prescriptions: If you are up to date with your appointments, ask the pharmacy to send a refill request to the office.

What if I am switching pharmacies?
​
Ask your current pharmacy to transfer the information to the new pharmacy.

Records
​
How can I get a copy of my records?
You may download the last 3.8 years from your patient portal at no charge. Anything beyond that patients must fill out a records request form. It and can be emailed or faxed to the office. It may take up to 7-10 business days to complete your request.

Is there a charge for records?
If you are requesting your records for yourself, these records may incur a charge, depending on the number of pages. We collect a $50 deposit to count the pages. There will be a prepayment fee (Pursuant to the Florida Administrative Code 21m-26.003 of $1.00 per page for the first 25 pages and $0.25 cents thereafter.

Balances

What does it mean to have a “no show” appointment?
​
This means that you scheduled an appointment which you did not show to the office for.
​
What are the fees for No Show appointments?
If it was a regular office visit or annual exam: $50
​​
Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. Therefore, patients who do not show up for their appointment without a call to cancel at least 48 hours before the appointment time will be considered as NO SHOW. This fee must be paid in full prior to your next appointment. Thank you for your understanding and cooperation as we strive to best serve the needs of all our patients.

How do I pay any balance I have?
There are multiple ways to pay off a balance that you may have with our office, including those for records and no show appointments.
